Request for Assistance

Requests for funds need to be submitted in writing. Please include:

• Child's first and last name and age.
   
• The number of children in your family and their ages.
   
• The amount of money you are applying for and how it will be used.
   
• Please describe your current family income:
  • Are you a single or two-parent family?
  • Working, on disability or social assistance, etc.
     
• Parent's name, address and daytime and evening phone numbers
   
• Where else you have tried to obtain funding

Once your request has been received, it will then go to the Board of Directors for a decision. When that decision is reached, you will be contacted. This process can take up to three to four weeks, although it is often less. In the event that your request is approved, you will be asked to contact the sports association/camp or company and have them fax us an invoice to The Max Keeping Foundation at 613-274-4301. The Foundation will then send the cheque directly to the company, camp, etc.

The Foundation has a maximum grant of $500, and may assist a family in two separate years. A written request is required each time.
